Acknowledgement Registration

Eligibility:
1.The putative father
2.The acknowledged child: the child may apply if the putative father fails to do so.
3.An interested party (when the preceding items do not apply)
4.A proxy, who shall provide legitimate reasons as approved by a household registration office
Required documents:
5.For voluntary acknowledgement: a written acknowledgement of paternity signed by both of the biological parents. In case that the putative father claims paternity without the biological mother, he shall present a paternity test report bearing the child’s photo and seals on the adjacent edges of consecutive pages.
6.For paternity established through a court order: a written judgment and a certificate proving that the court judgment has become final and binding.
7.For acknowledgement deemed through child-rearing: evidence of such child-rearing by the putative father.
8.The personal seal (signature acceptable), household certificate and national ID card of the putative father, as well as the household certificate and national ID card (if applicable) of the child. 9.A written agreement on the surname borne by the child if the child assumes the father’s surname
10.The proxy on behalf of an applicant shall present the power of attorney, and his/her national ID card and signature (or personal seal).
11.Documents prepared in a foreign country shall be certified by an R.O.C. embassy, consulate office, representative office, or any other institutes authorized by the Ministry of Foreign Affairs. Documents prepared in China shall be certified by the Straits Exchange Foundation. As for documents prepared in a foreign language, it is required to submit a Chinese counterpart and have it verified by an R.O.C diplomatic mission or certified by a domestic notary.
