I.Eligibility:
1.An adopter or an adopted person
2.An interested party (when the preceding item does not apply)
3.A proxy
II.Required documents:
1.A written approval and certificate proving that the court ruling has become final and binding
2.The national ID card, signature (or seal) of the applicant; and the household certificates and national ID cards of both the adopter and the adopted person. In case the adopted person has a national ID card, it is necessary to submit one photo taken within the past two years and pay NT$50 dollars as the processing fee for card renewal. 3.A written agreement on whether the adopted person will assume the adoptive father’s or the adoptive mother’s surname, or keeps the original surname unchanged
4.The proxy on behalf of an applicant shall present the power of attorney, and his/her national ID card and signature (or personal seal).
5.Documents prepared in a foreign country shall be certified by an R.O.C. embassy, consulate office , representative office, or any other institutes authorized by the Ministry of Foreign Affairs. Documents prepared in China shall be certified by the Straits Exchange Foundation. As for documents prepared in a foreign language, it is required to submit a Chinese counterpart and have it verified by an R.O.C diplomatic mission or certified by a domestic notary.
