Birth Registration

Applicants for birth registration shall be biological parents, grandparents, heads of households, cohabitants or foster parents. The application shall be filed by presenting the following documents at the household registration office of the intended place of residence:
1.Birth certificate (original coy required)
2.Household certificate, national ID card and personal seal (signature acceptable) of the applicant. The date of marriage or cohabitation of the parents must also be provided.
3.The proxy on behalf of an applicant shall also provide a power of attorney, and the national ID card and the proxy’s personal seal (signature acceptable).
4.Written agreement on the child’s surname (also applicable to foreign-born children). In case that the child bears the foreign parent’s surname, the name should be in line with the conventions in Taiwan, unless such agreement has been appended to the birth certificate. The agreement shall be stamped or signed and entered into by both parents of the newborn.
5.The birth registration of a dependent child requires documents issued by a police office/department and his/her photo for verification.
6.For an applicant as an interested party, he/she shall provide documents to prove his/her stance.
