The goal of Long-Term Care 2.0 is to establish a high-quality, affordable, and universal long-term care service system, realizing aging in place. Primary prevention functionality is optimized toward the front end and linked to preventive care. On the back end, multi-target community-based support services are provided for transfer to palliative care at home.
