In our lives, we have to make countless decisions; however, have you ever thought that we could not decide the look we were born, yet we can prepare ourselves for the day we leave for another world?
When facing the end of life, in addition to maintaining all the treatment until death, we can make the following decisions for ourselves: agreeing to donate our organs and accepting the hospice palliative care.
All hospitals and health clinics in Changhua County provide “The Hospice Palliative Care Consent Form” and “Organ Donation Consent Form” at Public Service Area. Everyone can take the consent forms at the service desk and sign!
